what are exosomes + microneedleing

Exosomes are nanoparticles released by nearly all cells in the human body, and skin cells have their very own kind of exosomes. They contain various lipids, proteins, amino acids, peptides, growth factors, and genetic material. 

Exsome therapy involves the application of topical exosomes (in the form of what looks and feels like a serum or gel) after treatments like resurfacing lasers or microneedling. I like to think of exosome therapy as the cherry on top of in-office treatments because it can only be done in conjunction with one — never on its own. 

That's because, although exosomes are nanoparticles, they cannot penetrate skin on their own. They need those disruptions in the superficial layer of skin to start communicating with skin cells. Otherwise, they'd just sit on the surface of your skin doing nothing.  This is also the reason why over-the-counter serums and creams aren't spiked with exosomes just yet.

Microneedling is a minimally invasive procedure that involves applying a microneedling stamp to very fine needles to the skin. The needles create tiny channels (microscopic injuries) in the skin that trigger the skin’s natural healing response. As a result, microneedling induces the formation of new collagen and elastin in the skin. That’s why it is also called collagen induction therapy. By promoting the formation of essential skin proteins collagen and elastin, microneedling can help reduce the appearance of stretch marks. The technique involves using different needles for different purposes. Stretch marks treatment requires the use of slightly longer microneedles that can penetrate midway through the dermis.

As we get older, our skin cells start to atrophy, and our bodies produce fewer exosomes — similar to how we experience a decrease in collagen production.  This leads to fine lines, wrinkles and loose or sagging skin.  Exosome therapy allows new exosomes to come in to help wake up inactive skin cells and older exosomes so they can get back to work to repair, strengthen, and rejuvenate skin.

  • The length of the treatment can depend on the individual’s needs. Typically, you will only need 15 to 30 minutes to complete the process once you have sufficiently numbed.

  • SkinPen’s® variable needle depth allows for maximum versatility, making it safe and effective to use on many parts of the body, including the face, neck, décolletage, hands, knees and back. Talk to your SkinPen® provider to discuss your specific needs.

  • Because there are some side effects, you might want to give yourself 2-3 days before going back to your regular schedule. Especially for your first time. Everyone's skin responds differently. Most patients are fine to go back to work the following day with some redness.

  • Most patients report minimal pain and the slight vibration of the SkinPen® during the treatment. The needles used are very sharp and fine, allowing the benefit of puncturing your skin without the feeling of any tugging or pulling. To manage any discomfort, we will use a topical cream applied to the treatment area for 10-15 minutes.

  • The total healing time depends on each individual. Most patients have some slight swelling and redness/pinkness (like a mild to moderate sunburn) for two to four days following the microneedling procedure. Some people may see their skin return to normal in about 24 hours.

  • Your skin will continue to improve over the weeks and months following your treatment. Because microneedling treatments stimulate your body into producing collagen, and elastin, as well as new skin cell and tissue growth, you need to give your body time to produce all of these great regenerative elements. The natural repair process of your own skin continues to progress through the course of several weeks. Damage that has occurred over an extensive period of time won’t disappear immediately. You may not see the final result for up to six months following your final session.

  • You should not get SkinPen microneedling if you are pregnant or breastfeeding, if you have low platelets or a bleeding disorder, if you have an active sunburn, infection or open lesions in the treatment area. Patients on “anti-clotting” medications (like Aspirin and other blood thinners) or who have certain autoimmune disorders should discuss treatment with their prescribing physicians.

    You are not a candidate if you are currently on Accutane or have been in the last 6 months.

  • This is the fun part! After being treated with SkinPen microneedling, your skin will be refreshed, revitalized, and enhanced. before you schedule

  • Avoid treatments that may cause skin irritation to the treatment area in advance of your treatment and Isotretinoin (Accutane) 6 months prior to treatment.

  • Avoid sun exposure 1 week prior to treatment. Excessive tanning of any sort (laying out, tanning beds, and artificial tanning lotions) is not recommended in the treated areas during the entire course of the treatment.

  • Avoid Botox 1 week before and after treatment.

  • Report any facial surgery in the last year prior to scheduling treatment.

  • Stop taking NSAID medicines such as Aspirin, Ibuprofen, and Naproxen since they interfere with normal blood clotting. Check medication labels for the active ingredient.

  • You may take Acetaminophen (Tylenol) as needed for pain not to exceed 2500 mg a day.

  • Discontinue any irritant type of topical products or treatment products 24 hours prior to treatment such as Alpha Hydroxy Acids, Beta Hydroxy Acid, Retinol (Vitamin A), Vitamin C (in a low pH formula), scrubs, or anything perceived as ‘active’ skincare.

  • No prolonged sun exposure to the area being treated. Sunburned skin cannot be treated.  Allow 48 hours after spray or self-tanning lotions prior to procedure.

  • Shave any hair-bearing body surfaces the day prior to treatment.

  • Shower as usual.

  • Do not apply any lotion, powder, oils, or other products to treatment areas.

  • Cleanse the area and do not apply make-up if you plan to treat the face/neck area. Wear accessible clothing for the area being treated. If treating the body, wear loose-fitting, comfortable clothing.

  • Notify Collagen Project of any active breakouts or open lesions. 

  • If you have long hair, bring a hairband to pull it back.

what to expect during your treatment

  • Your appointment will last about 60 minutes.

  • When you arrive, you will review and sign your consent forms.

  • You will be taken to an exam room and have photographs taken.

  • We will clean your skin and apply a topical anesthetic to the treated area.

  • You will need to let the numbing cream sit for 10-15 minutes. 

  • Once you are numb, your treatment will begin.

how to care for your skin after treatment

day of treatment

  • Avoid heat (sun, sweat, hot shower, etc.)

  • Treat area like a wound- do not apply any products unless recommended by Collagen Project, and keep the area clean (avoid excessive touching, contact with pets, etc)

day 1-3 after treatment

  • CLEAN: Use a soothing cleanser or face wash with lukewarm water to cleanse the face and gently pat dry the treated skin until it heals. Always make sure that your hands are clean when touching the treated area. Avoid any scrubs or exfoliants.

  • HYDRATE: Use a gentle moisturizer or healing ointment on the treatment area until the skin heals.

  • MAKEUP: Can be applied 48 hours after the treatment. Clean makeup brushes prior to use.

  • PROTECT: At 48 hours after the procedure, you should apply a broad spectrum UVA/UVB sunscreen with an SPF30, AT ALL TIMES when you will be outside.
